APPLICATION FORM
GENERAL EXPLANATIONS
The Article 11 of the Law on the Protection of Personal Data No. 6698 (“Data Protection Law”), entitles personal data holders as defined in the Data Protection Law as data subjects (hereinafter referred to as “the applicant”) to file applications regarding processing of their personal data.
In accordance with the first paragraph of Article 13 of the Data Protection Law, the applications must be in writing or in any other means determined by the Personal Data Protection Authority (“the Authority”) to our Company which is the data controller.
Within this frame, any applications to our Company, shall be communicated printing out this form and;
- Via application in person,
- Through public notary,
- By signing with the “Secure Electronic Signature” defined in the Electronic Signature Law No. 5070 and sending it to our e-mail address,
- If any, via e-mail that you have previously notified to our Company and registered in our Company’s systems.
The table here below gives information as regards the written application channels and the methods in which written applications shall be communicated to our Company.

We will respond within 30 (thirty) days from the date your request is received by us according to the nature of the claim, in accordance with the second paragraph of Article 13 of the Data Protection Law. Our answers will be delivered in written or electronic form in accordance with the provisions of Article 13 of the related Data Protection Law.
